Minimal Anxiety (score between 0-7)
Mild Anxiety (Score between 8-14)
Moderate Anxiety (score between 15-20)
Severe Anxiety (score between 21-26)

If you are worried about any of your scores, it can be helpful to talk to your parents or doctor or get support from your school’s guidance counsellor.

1.Do you worry a lot about the future or bad things happening (for example, illness, getting hurt or sick, going to a match)?
2.Do you often feel restless or on edge?
3.Do you have trouble sleeping because your mind is racing with thoughts?
4.Do you experience a lot of headaches or stomach aches?
5.Do you often experience nightmares?
6.Do you often feel uncomfortable in social situations or when talking to unfamiliar people (for example, going to a party, meeting new people)?
7.Do you spend a lot of time worrying about what other people think about you?
8.Do you spend at least an hour a day repeating things, such as washing, checking, arranging, or counting?
9.Do you spend a lot of time trying to make things perfect?
10.Do you have sudden rushes of intense fear or anxiety?
11.Are you afraid to be alone or do things on your own?
12.Do you have a strong fear of specific situations or things, such as blood, needles, heights, water, small spaces, flying, animals, or insects?
13.Do you often avoid going places or doing things because you feel anxious?
